15.04.25

The Integumentary System

The skin is one of the largest organs in the human body, with many functionalities One of the most important being a barrier to protect the body from external factors.

The skin is vulnerable to damage from trauma, sunlight, microbes and pollutants in the environment.

Common Dermatoses

Skin conditions include pathology affecting nails and hair, they make up one of the most common presentations in Australian general practice.

Affecting people’s mental health and quality of life

Common skin conditions are not limited to but include: Eczema; atopic, contact and seborrheic, Psoriasis, Acne, Rosacea, Urticaria, Fungal infections.

Atopic Dermatitis

What is Eczema?

Eczema is an inflammation of the skin, usually causing itching
and sometimes accompanied by crusting, scaling or blisters.

What Causes it?

Eczema or atopic dermatitis is caused by a combination of immune system activation, genetics, environmental triggers and stress.

Further causes include; poor skin barrier, skin and gut microbiome dysbiosis, climate, dietary and external irritants.

Some examples of these Environmental triggers are things such as soaps, chemicals, perfumes, pollen)

Other forms you can have are:

• IgE-mediated allergy. Allergies to micro minerals
• Poor balance of Microbial colonization.
• Mental health/ high stress.

Scratch and Itchy Cycle

The itch-scratch cycle: this is a continuous cycle that is impacted by the sensation of itching, the presence of eczema & dry skin resulting in damage and thus a further broken skin barrier and the
the cycle can continue.

Patients scratch, which causes more inflammation. The itch causes the scratch, and the scratch causes more inflammation with more rash.

This sensation of itch in eczema occurs when inflammation causes free nerve fibers to stimulate nerve endings called C fibers in the top layer of the skin (the epidermis).

  • Immune cells in the deeper layers of the skin send inflaming signals to the surface causing an itchy rash
  • Scratching breaks down the outer layer of the skin allowing allergens and germs to get in
  • Damaged skin, in response the immune system continues to send signals to the surface causing more redness and inflammation
  • Release of inflame signals, more scratching = more breakdown and thus the cycle start

What makes it worse – dry skin, temperature and changes in temp, overheating, sweat, certain clothing, irritants.

Maintaining Good Skin Integrity

Clean: Ensuring regular washing of gentle soap-free formulas is a pinnacle for every stage of Atopic
Eczema, cleaning prevents infection on already fragile and compromised skin. Try recommending a
gentle SLS and SLES Cleanser enables the patient to wash without further irritation and
inflammation of the epidermis.

Apply: Allopathic medications and topical steroids generally opt for application 1-2 x days;
leaving itching and irritation unsupported. When co-prescribing topical
herbal creams opt for the patient to feel symptomatic relief while reducing itching and irritation,
thus, reducing the itch-scratch cycle.

Maintain: Moisturisers work as the most important basic skin treatment for recovery regardless of
the severity and offers relief when in an active and nonactive eczema faire

So how do you do this and what can you recommend on the shop floor?
